Practice and Patient's Agreement

Liffock Surgery

  • Patients will receive courteous, respectful and confidential care. 
  • Urgent problems will be dealt with as quickly as possible by whichever doctor is available

  • To provide you with emergency care when required.

  • You will be given the opportunity to have a routine appointment with the doctor of your choice whenever it is possible.
  • You will be seen as near to your appointment time as is possible.
  • You will be given an explanation if waiting times are running late.
  • Advice and/or prescriptions will be made as necessary.
  • The practice will give you information about the services it offers, how to access them, and tips for staying healthy.
  • We will ensure the practice has full disabled access.
  • To refer you on when necessary to the appropriate consultant

  • The doctor is here to treat and counsel you. It is crucial that you comprehend the information provided to you by the doctor. Please request further information if you have any questions about your treatment

Patients

  • Patients are responsible for being kind and respectful to the employees at our practice.

  • Only request an emergency appointment only when you feel it is medically necessary

  • Please inform us as early as possible if you cannot keep your appointment so that we can offer it to someone else.

  • Requesting a home visit should only be done in extreme cases. Lack of transport is not an acceptable reason. It is much preferred to come to the surgery where equipment and notes are available and treatment can begin immediately. 

  • Please do not call the out-of-hours service except in an emergency.

  • Please inform the receptionist if you change your name, address or telephone number

  • To let us know if you are unsure about the treatment we are offering you.

  • To be on time for appointments, and to make a separate appointment for each person wishing to see the doctor.

  • To take medicine as directed by the instructions and to only ask for a repeat prescription when needed.

Zero Tolerance Policy

The doctors and staff put in a lot of effort to give our patients high-quality care. They must be able to do so in a safe setting. Our staff come to work to care for others, not to be victims of violence, threatening behaviour or physical, verbal or racial abuse/discrimination. Unsubstantiated statements on social media about the practice or members of our staff will be treated in the same way as verbal abuse.

A patient will be removed from the practice list if they engage in any abusive or aggressive behaviour toward a member of the practice team, whether it occurs on the premises or outside.
